This systematic review and meta-analysis of the literature supports the recommendation that scleral buckling surgery be performed within three days to optimize visual outcomes. Patients who had surgery within three days after macular detachment had a statistically significant better chance of reaching a final visual acuity of 0.4 logMAR or better, with an odds ratio for ≤3 days versus 4 days to 7 days of 2.86, and an odds ratio for ≤3 days versus >3 days of 3.09. The Centers for Medicare and Medicaid Services is delaying payments to health care providers that were scheduled to begin Jan. 1 under the 2015 Medicare Physician Fee Schedule. The agency says the hold is being implemented while it makes technical corrections to the payment rates that were erroneously published in the fee schedule. CMS indicates that it expects to have revised payment rates available no later than Jan. 20.
